You can choose a basic form, or we can build a custom form using your logo, colors, in any size you need, with any type of questions you require. | Customize your form by adding a logo to increase brand-name recognition. If you provide us with a copy of the logo, we can resize it and place it anywhere on the response form. |

| Choose any font for the headers on your form (the ones at right are just a small sample of different kinds of fonts). Usually it is best to keep the font style consistent with those already existing on your site. |

| Various types of textures can be used in the background on your form; you may also choose a solid color, a pattern already used on your page, or no background at all (white). |

| It is also important to decide how big the form should be – a fixed width, or a certain percentage of the viewer's browser window?
